Scheduling

Welcome to the Center for Oral Maxillofacial Surgery and Dental Implants. We are dedicated to not only providing the very best oral surgery services, but we make every effort to go above and beyond to help you feel relaxed, comfortable, and well informed.

The Center for Oral Maxillofacial Surgery and Dental Implants is open Monday through Friday from 8:00 am until 4:00 pm. We will schedule your appointment as promptly as possible, however we require a referral form from your dentist prior to scheduling. Post-op exams and some consultations may occasionally be scheduled on Saturdays from 8:00 am until 10:00 am.

We strive to stay on schedule to minimize your waiting, however some surgical services may require unexpected additional time to allow for appropriate care of all of our patients. Emergency cases may also sometimes cause delays. We appreciate your understanding and patience.

Please call our office at 608-819-8760 (West Side) or 608-240-0863 (East Side) with any questions, or to schedule an appointment. It is helpful to have your insurance cards handy when you call.

Your First Visit

Your initial appointment may consist of a consultation explaining your diagnosis and treatment options. A complex medical history or treatment plan may require a consultation prior to scheduling surgery.

Please assist us by providing the following information at the time of your first visit:

  • Your Medical and Dental insurance information
  • A list of current or prior medical conditions, including diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.
  • A list of the medications you are presently taking (heart medications, aspirin, anticoagulant therapy, etc.)
  • Referral forms or X-rays given to you by your dentist or physician. We offer an online portal [add link] which allows your dental office to transfer x-rays electronically. You may also pick up the x-rays and bring them to our office. If additional films are necessary, they can be taken at our facility.

If you have not already filled out the Online Patient Registration and Health History Forms, please arrive early to allow time to complete them.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

If you will be undergoing IV anesthesia, please have nothing to eat or drink six to eight hours prior to your surgery. You will also need an adult to drive you home. We require that the driver remain in the office for the duration of the appointment if the patient is being sedated. Please see our Before Anesthesia Care Guide for additional information.

Insurance and Financial Information

The Center for Oral Maxillofacial Surgery and Dental Implants is dedicated to delivering the finest oral surgery care at the most reasonable cost to our patients. To accomplish this, we work with you to maximize your insurance reimbursement for covered procedures and are happy to submit claims on your behalf.

We are Providers for the following Insurance Companies:

If you do not see your insurance listed and need to undergo a medical procedure such as removal of wisdom teeth, a biopsy, or corrective jaw surgery, please contact us to see if you have in-network coverage with our office. Dental procedures such as removal of removal of erupted teeth or dental implants are not typically covered by medical insurance and are typically filed under any dental insurance you may have. We are always happy to work with out of network patients to meet their treatment needs as well.

Please bring both your medical and dental insurance cards and information with you to your appointment so we can expedite reimbursement. The amount we collect on the day you are in our office is only an estimate of your copayment — this estimate is based on information received from your insurance company and is not a guarantee of benefit or payment. Providing our office with accurate medical and dental insurance information will help us expedite your insurance determination of benefits. For your convenience, we accept Visa, MasterCard, American Express and Discover. You may also Pay Online. A monthly payment plan can be arranged through Care Credit .

No Surprise Billing Act

Effective 1/1/2022

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You are also protected from balanced billing (sometimes called “surprise billing”) in certain circumstances. When you get emergency care or are treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from balance billing. OMS practices are not included in IFRs definition of health care facility.

Privacy Information

Each time you visit the Center for Oral Maxillofacial Surgery and Dental Implants for care, a record of your visit is made. This record usually contains identification and financial information as well as symptoms, diagnoses, test results, a description of an oral examination and a treatment plan. This record is often referred to as your “medical record,” or “health information,” and includes information contained in paper as well as electronic records.
